Zusammenfassung
Bei 25 akromegalen Patienten (13 Männer und 12 Frauen) sowie 15 stoffwechselgesunden Normalpersonen wurde mit dem Hypronosticon®-Test die renale Hydroxyprolinausscheidung überprüft. Gleichzeitig wurden der Phosphat- und Calcium-Spiegel im Serum sowie die Phosphat- und Calcium-Ausscheidung im Urin gemessen. Bei akromegalen Patienten im aktiven Stadium der Erkrankung fand sich eine signifikant höhere Ausscheidung an Hydroxyprolin im Harn (32,55 ± 8,68 mg/24 h · m2) als bei Normalpersonen (12,12 ± 2,33 mg/24 h · m2). Sie schieden signifikant mehr Hydroxyprolin aus als die behandelten Patienten, deren Erkrankung sich aufgfund normaler Basalwerte und guter Supprimierbarkeit des STH unter oraler Glucosebelastung als nicht mehr floride erwies (13,49 ± 4,01 mg/24 h · m2). Akromegale Patienten, bei denen kein Therapieerfolg nachweisbar war, zeigten dagegen neben den erhöhten STH-Werten basal eine gesteigerte renale Elimination von Hydroxyprolin (31,23 ± 16,61 mg/24 h · m2). Die Hydroxyprolinausscheidung im Harn gibt somit als metabolischer Parameter zusätzlich Hinweise auf den Aktivitätsgrad der Akromegalie.
Summary
Urinary excretion of hydroxyproline was examined with the Hypronosticon® test in 25 patients with acromegaly (13 men, 12 women) and 15 healthy persons. At the same time, plasma levels and urinary excretion of phosphorus and calcium were also measured. Patients with active acromegaly had a significantly higher urinary hydroxyproline output (32.55 ± 8.68 mg/24 h · m2) than healthy persons (12.12 ± 2.33 mg/24 h · m2). They also excreted considerably more hydroxyproline than patients with successfully treated acromegaly. Effective treatment was proved by STH levels and good suppressibility by 100 mg glucose. Unsuccessfully treated patients, on the contrary, had a raised urinary excretion of hydroxyproline (31.23 ± 16.61 mg/24 h · m2) and high fasting plasma levels of somatotropic hormone. Hydroxyprolinuria proved, therefore, of value in assessing the development and treatment of acromegaly.
