Zusammenfassung
Von 1945 bis 1970 wurden 308 maligne Knochentumoren diagnostiziert: 89 Plasmozytome, 66 Osteosarkome, 47 Chondrosarkome, 32 Fibrosarkome, 26 Retikulumzellsarkome, 23 Ewing-Sarkome und einige sehr seltene Geschwülste. Beim Plasmozytom konnte von 1957 bis 1970 eine Zunahme der Fälle um das Fünffache festgestellt werden. Das männliche Geschlecht ist mit 161 Fällen etwas häufiger von bösartigen Knochengeschwülsten befallen als das weibliche (132 Fälle). Am häufigsten kommen diese Tumoren im zweiten und siebten Lebensjahrzehnt vor. Einige Geschwülste finden sich gehäuft bei Jugendlichen (Ewing-Sarkom, Osteosarkom), andere mehr im höher en Lebensalter (Plasmozytom). Häufigste Lokalisation ist die Knieregion. Unter Zugrundelegung der röntgenologischen und histologischen Befunde lassen sich die verschiedenen Geschwülste genau klassifizieren, was für die Therapie und Prognose von großer Bedeutung ist. Häufigste klinische Symptome sind Schmerzen und eine lokale Schwellung. 77,8% aller Patienten gingen innerhalb der ersten beiden Beschwerdemonate zum Arzt; bei nur 29,6% der Patienten wurde jedoch innerhalb der ersten 3 Krankheitsmonate die Diagnose »maligner Knochentumor« gestellt. Die Prognose dieser Geschwülste ist schlecht: 87,3% der Patienten starben in den ersten 3 Jahren nach der Diagnosestellung.
Summary
Between 1945 and 1970 a total of 308 malignant bone tumours were diagnosed: multiple myeloma (89), osteosarcoma (66), chondrosarcoma (47), fibrosarcoma (32), reticulum-cell sarcoma (26), Ewing's sarcoma (23), and single cases of various very rare tumours. 15 tumours were »semi-malignant« and excluded from further analysis. There were 161 males. The number of multiple myelomas increased fivefold between 1957 and 1970. The most common decade was the second and seventh. Some tumours were particularly frequent among young people (Ewing’s sarcoma, osteosarcoma), others more so at a higher age (multiple myeloma). The knee region was the most frequent site. The various tumours can be exactly classified by radiological and histological means, which is of great importance in treatment and prognosis. Pain and local swelling were the most frequent symptoms. 77.8% of all patients consulted a doctor within the first two months after onset of symptoms, but only in 29.6% was the diagnosis of malignant bone tumour made within the first three months. The prognosis of these tumours is very grave: 87.3% of patients died within the first three years after diagnosis.
