IgM-Paraproteinämie bei lymphoblastischem Lymphosarkom im Kindesalter

Zusammenfassung

Bei einem 12œ Jahre alten Knaben, der an einem lymphoblastischen Lymphosarkom litt, war von Beginn der klinischen Beobachtung an, im Verlauf der Erkrankung zunehmend, im Serum ein monoklonales Paraprotein vom IgM-χ-Typ nachzuweisen. Gleichzeitig gingen die IgG- und IgA-Konzentrationen auf pathologisch erniedrigte Werte zurück. Therapeutische Maßnahmen hatten keinen Einfluß auf den klinischen Verlauf. Trotz der eindeutig pathologischen Befunde im Immunglobulinsystem waren die Ergebnisse der Folienelektrophoresen während der Beobachtungszeit weitgehend unauffällig. Die Diagnose »Paraproteinämie« wurde anhand der immunelektrophoretischen Analyse und der Ergebnisse von Gelfiltrationen über Sephadex G 200 gestellt.

Summary

A monoclonal paraprotein of the IgM-χ-type was demonstrated in 12œ-year-old boy who had a lymphoblastic lymphosarcoma, the levels being raised from the onset of clinical observation and gradually increasing in the course of the disease. At the same time, IgG and IgA concentrations fell to abnormally low values. Therapeutic measures had no influence on the clinical course. Despite the definite abnormal findings in the immunoglobulin system the results of foil-electrophoresis were largely unremarkable. The diagnosis »paraproteinaemia« was made on the basis of immuno-electrophoretic analysis and the results of gel-filtration with Sephadex G 200.
