Zusammenfassung
In den Jahren 1955-1972 wurde bei 20 Kranken eine Strahlenpilzerkrankung festgestellt. Die cervicofaciale Form lag bei sechs, die thorakale bei neun und die abdominale bei fünf Kranken vor. Aufgrund des klinischen Bildes war nur in zwei Fällen von cervicofacialer Aktinomykose die Diagnose gestellt worden. Der bakteriologische Nachweis des Erregers gelang nur in sechs Fällen. Die übrigen Patienten waren im wesentlichen unter der Diagnose eines Tumors, einer Pneumonie oder eines unspezifischen Abszesses bei unterschiedlicher Symptomatik in der Klinik aufgenommen worden. In diesen Fällen wurde die Krankheit erst mit Hilfe pathohistologischer Untersuchungen an Operationspräparaten erkannt. Die diagnostischen Schwierigkeiten machen auch heute noch die Aktinomykose zu einer »chirurgischen« Infektion. Darüber hinaus erfordern therapeutische Gesichtspunkte häufig operatives Eingreifen, da die Antibiotikatherapie zwar eine unerläßliche, oft aber nicht ausreichende Voraussetzung der Heilung darstellt.
Summary
Actinomycosis was diagnosed in 20 patients between 1955 and 1972. Six cases had the cervicofacial, nine the thoracic and five the abdominal form of the disease. The clinical picture had led to a correct diagnosis in only two cases of cervicofacial actinomycosis. Bacteriological demonstration of the fungus was possible in only six cases. The remaining patients had mainly been admitted with the diagnosis of tumour, pneumonia or nonspecific abscess with varying symptomatology. In these cases the disease was diagnosed only after histological examination of tissue specimens obtained by surgery. Due to diagnostic problems actinomycosis is still first encountered by the surgeon. Beyond that, surgery is often required for therapy as treatment with antibiotics is obligatory but not always sufficient for cure.
