Zusammenfassung
Neun Patienten mit endokriner Ophthalmopathie und vier Personen ohne Exophthalmus, aber mit positiver Reaktion auf exophthalmus-produzierenden Faktor (EPF) im Serum wurden mit D-Thyroxin behandelt. Wie die Auswertung der Ergebnisse zeigt, bestehen zwischen endokriner Ophthalmopathie, exophthalmus-produzierendem Faktor und Struma keine festen Beziehungen. Trotz ausreichender Bremsung der hypophysären Abgabe von exophthalmus-produzierendem Faktor durch D-Thyroxin kann ein Exophthalmus entstehen, während die EPF-Reaktion im Serum unverändert positiv bleibt. Nicht in allen Fällen verschwindet der Faktor nach Behandlung mit D-Thyroxin aus dem Serum. Selbst bei negativem Ausfall des biologischen EPF-Nachweises kann sich die Behandlung mit D-Thyroxin günstig auf eine doppelseitige Ophthalmopathie auswirken. Bei Patienten mit positiver EPF-Reaktion ohne endokrine Ophthalmopathie finden sich oft Bedingungen, die mit besonders starker TSH-Aktivität einhergehen. Extrahypophysäre Faktoren dürften bei der Pathogenese der endokrinen Ophthalmopathie ebenfalls eine Rolle spielen.
Summary
D-thyroxine was administered to nine patients with endocrine ophthalmopathy and four without exophthalmos but with positive reaction to exophthalmos-producing factor (EPF) in serum. No definite correlation was demonstrated between endocrine ophthalmopathy, EPF and goitre. Although D-thyroxine satisfactorily inhibits hypophyseal release of EPF, no exophthalmos will occur as long as the EPF reaction on serum remains positive. The factor does not disappear from serum in all patients receiving D-thyroxine. Even if the test for EPF is negative, treatment with D-thyroxine may cause improvement of bilateral exophthalmos. In patients with positive EPF reaction but no endocrine ophthalmopathy there are often other abnormal conditions associated with a particularly high TSH activity. Furthermore, extrahypophyseal factors probably also play a role in the pathogenesis of endocrine ophthalmopathy.

Acerca de la importancia patogenética de los factores hipofisarios en la oftalmopatía endocrina
Nueve pacientes con oftalmopatía endocrina y cuatro personas sin exoftalmos, pero con reacción positiva al factor productor del exoftalmos (EPF) en el suero, fueron tratados con tiroxina D. Como muestra la valoración de los resultados, no existen relaciones fijas entre la oftalmopatía endocrina, el factor productor del exoftalmos y el bocio. A pesar de la suficiente acción de freno de la tiroxina D sobre la emisión hipofisaria del factor productor del exoftalmos puede desarrollarse un exoftalmos, mientras que la reacción al EPF sérico sigue siendo invariablemente positiva. No en todos los casos desaparece del suero el factor después del tratamiento con tiroxina D. Incluso siendo negativo el resultado del análisis biológico del EPF, el tratamiento con tiroxina D puede producir un efecto favorable sobre una oftalmopatía bilateral. En los pacientes con reacción positiva al EPF sin oftalmopatía endocrina se encuentran con frecuencia condiciones que van acompañadas de una actividad especialmente intensa de la TSH. Igualmente, los factores extrahipofisarios podrían jugar un papel en la patogénesis de la oftalmopatía endocrina.
