Kernaussagen
Der akute thoraxchirurgische Notfall kann in 4 Gruppen gegliedert werden: das akute thorakale Trauma (Polytrauma), die akute endoluminale Blutung, die Fremdkörperaspiration und der akute Pneumothorax, ggf. mit Spannungssymptomatik. In der Industriegesellschaft überwiegen Thoraxverletzungen infolge von Unfällen mit stumpfen Thoraxtraumen. Bei der Hälfte der polytraumatisierten Patienten findet man eine Beteiligung des Thorax.
Wichtig ist eine suffiziente Primärversorgung von Thoraxverletzungen, da das schwere Thoraxtrauma isoliert bei 5 % und bei polytraumatisierten Patienten bei bis zu 35 % die Todesursache ist. Der Stabilisierung der Vitalparameter (ABC-Regel) schließt sich eine schnelle orientierende Diagnostik (Thorax-CT, Bronchoskopie) und die primäre Notfallversorgung an. Therapie der Wahl ist in über 80 % der Fälle das Anlegen einer großlumigen Thoraxdränage.
Als besondere Herausforderung gilt neben der notwendigen Zusammenarbeit aller Fachdisziplinen (Anästhesie, Thoraxchirurgie, Herzchirurgie, Unfallchirurgie) die Schwierigkeit, dass der Zeitpunkt einer etwaigen Notfalloperation nicht verpasst werden darf.
Prinzipiell können alle Strukturen im Thorax betroffen sein. Besonderes Augenmerk muss auf die großen Gefäße und etwaige Verletzungen sowie eine Herzbeuteltamponade gerichtet werden. Die Thoraxdränage ist schnell und einfach einzubringen und verfolgt das Ziel, eine komplette Ausdehnung der Lunge durch die Mobilisierung von Blut (Hämatothorax) oder Luft (Pneumothorax) zu erreichen. Die Thorakotomie im Schockraum bleibt schwerstverletzten Patienten in Situationen vorbehalten, die keine Zeitverzögerung erlauben (Herzbeuteltamponade, schwere thorakale Blutung, Prävention einer Luftembolie, Durchführung der offenen Herzdruckmassage).
In jedem Fall muss der Patient nach Stabilisierung intensiv überwacht werden, und zwar möglichst in einem thoraxchirurgischen Zentrum unter Hinzuziehen der notwendigen Fachdisziplinen.
