Parenterale Chinin-Artesunat-Kombinationstherapie bei lebensbedrohlicher Malaria

Zusammenfassung
Anamnese und klinischer Befund: Eine 31-jährige Französin kongolesischer Abstammung kam nach einem Heimatbesuch zerebral eingetrübt und mit hohem Fieber zur Übernahme in unsere Intensivstation.
Untersuchungsbefund: Die Mikroskopie eines Dicken Tropfen und Blutausstriches zeigte Trophozoiten von Plasmodium falciparum mit einer hohen Parasitämie von 50 % und bestätigte den klinischen Verdacht auf eine unmittelbar lebensbedrohliche Malaria tropica.
Therapie und Verlauf: Es wurde sofort eine intravenöse Chininaufsättigung in einer Dosis von 20 mg/kg Körpergewicht (KG) über 4 Stunden eingeleitet, gefolgt von 10 mg/kg KG alle 8 Stunden. Da die Patientin zunehmend komatös wurde, wurde 6 Stunden später Artesunat in der empfohlenen Dosis von 2,4 mg/kg KG sofort, nach 12 und 24 Stunden und dann einmal täglich, hinzugefügt. Außerdem erhielt die Patientin Clindamycin 20 mg/kg KG/d i. v. Zur Korrektur der Anämie wurden 4 Erythrozytenkonzentrate bis zum Erreichen einer Hämoglobinkonzentration von 9,6 g/dl transfundiert. Unter dieser Therapie klarte die Patientin rasch auf, die Parasitämie sank innerhalb 24 Stunden auf 0,1 %. Bereits am 3. Tag waren keine Plasmodien im Dicken Tropfen und Ausstrich mehr nachweisbar.
Folgerungen: Der Fall zeigt die rasche Wirksamkeit einer parenteralen Artesunat-Chinin-Kombinationstherapie bei einer akut lebensbedrohlichen Malaria tropica mit außergewöhnlich hoher Parasitämie.
Summary
History and clinical findings: A 31-year old French woman of congolese origin was referred to the intensive care unit of our University Hospital after a visit to the Democratic Republic of Congo in a highly febrile and semi-conscious status.
Investigations: Microscopy of stained thin and thick blood films revealed Plasmodium falciparum trophzoites with a high parasitaemia of 50 % and confirmed the clinical suspicion of a life-threatening malaria.
Treatment and course: Immediate intravenous quinine with a loading dose of 20 mg/kg body weight was initiated followed by 10 mg/kg every 8 hrs. Since the patient lost consciousness, artesunate was added six hours later in the recommended dose of 2.4 mg/kg immediately, repeated after 12 and 24 hrs, followed by a daily dose of 2.4 mg/kg.
Furthermore, clindamycin at a dose of 20 mg/kg body weight / d i. v. was given. Anaemia was corrected by 4 erythrocyte concentrates until a haemoglobina concentration of 9.6 g/dl was reached. Now the patient rapidly recovered, and parasitaemia dropped to 0.1 % within 24 hrs. Already the following day blood films cleared completely from malaria parasites.
Conclusion: The case illustrates the rapid efficacy of a combined parenteral quinine-artesunate therapy in life-threatening falciparum malaria with an unusually high parasitaemia.
