Diagnostik

  • Die Indikation zur Hepatitis-B-Diagnostik (HBs-Antigen) ist breit zu stellen, u. a. bei Patienten mit erhöhten Leberwerten, mit Migrationshintergrund, bei Blutspendern, Schwangeren etc.

  • Bei jedem Hepatitis-B-Patienten muss mindestens einmal anti-HDV bestimmt werden, um eine eventuelle Simultaninfektion zu erkennen.

Therapie

  • Therapieindikation besteht bei fulminanter Hepatitis B (Quick < 50 %), chronischer Hepatitis B (> 10 000 Kopien/ml) und bei HBV-Leberzirrhose, wenn sie HBV-positiv ist.

  • Die Therapieindikation muss gut überdacht werden: Es ist einfach eine Therapie zu beginnen, aber schwierig, sie zu beenden.

  • Bei der Therapie sollte stets die Möglichkeit einer Behandlung mit IFN α bedacht werden (limitierte Therapiedauer mit der Möglichkeit der HBs-Serokonversion = ultimatives Therapieziel)!

  • Bei der Therapie mit Nukleos(t)idanaloga muss die Behandlung rechtzeitig angepasst werden, um Resistenzen verhindern. HBV-DNA ist alle 3 Monate zu kontrollieren, um rechtzeitig Resistenzen zu erkennen.

  • Bei immunsuppressiver Therapie sollte vorher an die Möglichkeit einer Hepatitis-B-Reaktivierung gedacht werden.
