Kongenitale melanozytäre Nävi – eine „Internet-Erkrankung”

Zusammenfassung

Der kongenitale melanozytäre Nävus (KMN) ist ein Krankheitsbild, über das viel Neues zu vermelden ist. So kamen erst in den letzten Jahren längst etablierte Dogmen ins Wanken, wie z. B. dass es sich bei KMN um beinahe obligate Präkanzerosen handele. Auch die mit größeren KMN gelegentlich einhergehende ZNS-Beteiligung (neurokutane Melanozytose) wird in vielen Lehrbüchern noch fälschlich als eine Erkrankung mit zumeist infauster Prognose behandelt. Eine differenziertere Betrachtung wäre wünschenswert, hierzu erforderliche wissenschaftliche Untersuchungen sind jedoch ganz wesentlich von der untersuchten Fallzahl abhängig. Daher haben für die relativ seltene Entität (größerer) KMN systematische Literaturanalysen einen hohen Stellenwert. Von wichtiger praktischer Bedeutung ist eine verbesserte Kommunikationsstruktur zwischen Betroffenen und Ärzten unter- und miteinander, wie sie sich über internetbasierte Informationsseiten und Selbsthilfegruppen entwickelt hat. Zu den in diesem Artikel zu besprechenden Neubewertungen der Situation von Patienten mit KMN hat der verbesserte Informationsaustausch über das Internet daher ganz wesentlich beigetragen

Abstract

In the last years, several new aspects have changed the image of congenital melanocytic nevi (CMN). This especially applies to the melanoma risk, which in the past was regarded as a general feature of CMN, regardless of their size. Another example of a paradigm shift is neurocutaneous melanocytosis (large CMN with CNS involvement), which was formerly thought to follow a fatal course in the majority of cases. Progress in the field of rare diseases depends on a synopsis of a considerable number of cases. Systematic literature analyses are needed to reveal inevitable biases produced by smaller case series. Moreover, new communicative structures, most importantly internet-based organizations, play a central role by linking patients and doctors together. The internet has therefore importantly contributed to the new perspectives on CMN which will be discussed below.
