Bei etwa 10 – 15 % der Schlaganfälle im Territorium der A. carotis interna wird eine ipsilaterale Karotisstenose gefunden, die mit einem erheblichen Risiko eines erneuten Schlaganfalls einhergeht. Das Karotisstenting (CAS) ist eine seit vielen Jahren etablierte Therapie, die eine der Thrombendarteriektomie vergleichbare perioperative Komplikationsrate aufweist und ebenso effektiv ist zur Schlaganfallsprophylaxe. Insbesondere jüngere Patienten scheinen von einem CAS zu profitieren. Die Kenntnisse und Erfahrung des Therapeuten und die verwendeten Materialien sind von großer Bedeutung für die Patientensicherheit. Für asymptomatische Karotisstenosen ist die klinische Wirksamkeit anzunehmen, mit einer prospektiv-randomisierten Studie jedoch noch nicht bewiesen.

Abstract

An ipsilateral stenosis of the internal carotid artery is found in 10 – 15 % of all ischemic strokes and indicates an increased risk of a second stroke. Carotid artery stenting (CAS) is a therapy that is established for many years. CAS reveals complication rates and long-term efficacy comparable to carotid endarterectomy (TEA). Especially younger patients seem to benefit from CAS. Abilities and experiences of the therapist and the choice of the techniques used are critical for patient safety. The efficacy of CAS for treatment of asymptomatic carotid stenosis is probable but still unproven in prospective-randomized trial.
