Guttapercha– vs. Sealer–Anteil in Abhängigkeit der Kanalinstrumentationstechnik einfach gekrümmter Wurzelkanäle

100 simulierte Wurzelkanäle in Epoxydharzblöcken wurden in 10 Gruppen à 10 Proben aufgeteilt. Die ersten 5 Gruppen wurden nach der Aufbereitung durch die laterale Kondensation, die anderen 5 Gruppen durch das One–Step–System abgefüllt. Die Aufbereitung erfolgte mittels 5 verschiedener Nickel–Titan–Systeme (FlexMaster, M–File, Mity Roto File, NiTi–TEE, ProTaper/ProFile). Anschließend erfolgte die Herstellung von Querschnitten 1,5 mm vom Apex entfernt. Diese wurden mithilfe eines Auflichtmikroskops mit integrierter Digitalkamera in der Vergrößerung 100 : 1 fotografiert. Die Auswertung erfolgte durch das Programm AutoCAD 2005. Vermessen wurde der prozentuale Anteil an Guttapercha und Hohlraum/Sealer im Wurzelkanalquerschnitt. In allen Gruppen wurde EZ–Fill als Sealer verwendet.

Während es in der Gruppe der kalten Guttapercha–Fülltechnik (laterale Kondensation) zu keinem signifikanten Unterschied (p > 0,05) im prozentualen Anteil des Hohlraums/Sealers bei den unterschiedlichen Aufbereitungsmethoden kommt, sind in der Gruppe der warmen Guttapercha–Fülltechnik (One Step) signifikante Unterschiede (p < 0,01) zu erkennen. Die mit lateraler Kondensation abgefüllten Kanäle haben einen Anteil von 25,21  %, die mit One Step einen von 9,22  % an Hohlraum/Sealer. Das beste Ergebnis (4,405  % Anteil Hohlraum/Sealer) konnte in der Kombination ProTaper/ProFile und One Step erreicht werden, das schlechteste ergab sich bei selbiger Aufbereitung und Obturation durch die laterale Kondensation (32,675  % Anteil Hohlraum/Sealer).

Ausgehend von der Annahme, dass es für die Langzeitdichtigkeit einer Wurzelkanalfüllung von Bedeutung ist, einen möglichst hohen Guttapercha– und einen nur geringen Hohlraum/Sealer–Anteil im Kanal sicherzustellen, ist in dieser Untersuchung das One–Step–System der lateralen Kondensation als Abfüllmethode überlegen. Zu beachten ist die qualitative Abhängigkeit der One–Step–Obturatoren von der Methode der Wurzelkanalaufbereitung.

A total of hundred simulated canals in plastic blocks were divided into ten groups of ten. After instrumentation the first five groups were then filled using the lateral condensation, the other groups were obturated using One Step. The groups were prepared with five different rotary NiTi systems (FlexMaster, M–File, Mity Roto File, NiTi–TEE, ProTaper/ProFile). After obturation the canals were sectioned horizontally 1.5 mm from the apex. Photographs of each section were taken using a microscope at a magnification of x 100 and the total area of the canal filled with guttapercha and voids/sealer were measured by AutoCAD 2005. Finally the ration of the guttapercha filled area and the voids/sealer area were calculated. All canals were coated with EZ–Fill sealer.

There was no statistical difference in the percentage of voids/sealer in the group of the lateral condensation (p > 0.05) but in the canals that were obturated by One Step (p < 0.01) between all shaping instruments. Root canals obturated with One Step contained significantly less voids/sealer ratio (9.22  %) than did those obturated with lateral condensation (25.21  %). The ProTaper/ProFile group obturated with One Step had the lowest voids/sealer ratio (4.405  %). The same preparation technique obturated with lateral condensation had the highest voids/sealer ratio (32.675  %).

The higher the percentage of guttapercha and the smaller the area of voids and sealer the better is the quality of root canal fillings. Maximizing the volume of guttapercha and leaving a very thin layer of sealer is preferred in modern endodontics because the sealer may shrink during setting and dissolve in time and produce leakage. One Step was found to be superior to the lateral condensation technique in terms of guttapercha and voids/sealer ratio. The quality of the One Step obturation technique depends on the canal preparation.
