Zur Problematik der Arteriographie der unteren Extremitäten

Zusammenfassung

Es wird über 120 Arteriographien berichtet, die nach rheographischer Voruntersuchung durchgeführt wurden. Aus der Gegenüberstellung beider Untersuchungsmethoden und Einbeziehung der Operationsbefunde und Resultate konrite eine Klarstellung ihrer Aussagekraft gewonnen werden. Bildet die Arteriographie die wesentliche Beurteilungsmöglichkeit für die Durchführbarkeit gefäßplastischer Operationen, so ist rheographisch mehr Einblick in funktionelle Verhältnisse, zum Beispiel bezüglich des Sympathektomie-Effektes, zu gewinnen. Die wertvolle Ergänzung beider Methoden bei verschiedenen Grenzfällen oder Erfassung von Frühfällen wird besprochen. Jedenfalls ist auf Grund der vorliegenden Ergebnisse festzustellen, daß nur die Kombination beider Untersuchungsmethoden einen maximalen diagnostischen Erfoig gewährleistet.

Summary

The authors report 120 arteriograms carried out after studies of the haemodynamics. By comparing the results of these examinations and the findings at operation it was possible to compare the values of each type of examination. Arteriography constitutes the best means for evaluating the operability, whereas haemodynamic studies provide a better understanding of blood flow, particularly with regard to the effect to be expected from sympathectomy. The methods supplement each other in difficult or early cases. From their experience the authors conclude that a maximum of diagnostic information can be obtained only by a combination of both methods.

Présentation de plus de 120 artériographies, qui ont été précédées d'une rhéographie. En comparant ces deux méthodes d'examens et en tenant compte du status opératoire et des résultats, il a été possible d'obtenir des précisions concernant leur valeur respective. L'artériographie constitue la méthode de choix pour l'appréciation de l'indication des opérations des plasties vasculaires; la rhéographie livre davantage de renseignements dans le domaine fonctionnel par exemple pour l'étude de l'effet d'une sympathectomie. Les auteurs démontrent comment leurs examens se complètent dans différents cas limites et par le dépistage des cas à leur début. Ces études illustrent en tout cas nettement que seule la combinaison de ces deux examens livre le plus de renseignements utiles à l'établissement d'un diagnostic.

Se informa acerca do 120 arteriografías obtenidas tras un examen reográfico preliminar. Cotejando ambos métodos de reconocimiento, y teniendo en cuenta lo revelado en la operación y los resultados, se ha puesto en claro su utilidad informativa. Así como la arteriografía constituye un importante medio para apreciar la practicabilidad de operaciones angioplásticas, la reografía instruye mejor respecto a factores funcionales, por ejemplo, al efecto de la simpaticectomía. Se comenta el gran valor complementario de ambos métodos en diversos casos limites, o en la apreciación de casos precoces. De todos modos, a base de los resultados conocidos, es evidente que sólo la combinación de estos dos métodos de éxamen garantiza un acierto diagnóstico máximo.
