Präoperative angiographische Darstellung eines Aneurysmas der Arteria gastro-duodenalis

Zusammenfassung

Bei einem 33jährigen Patienten, der seit 1/2 Jahr an ungeklärten lebensbedrohlichen kontinuierlichen Darmblutungen erkrankt war, konnte erstmalig durch selektive Angiographie präoperativ ein Aneurysma der A. gastro-duodenalis diagnostiziert werden. Alle sonstigen röntgenologischen Untersuchungsmethoden einschließlich der hypotonen Duodenographie, selbst der Serienetagenaortographie, versagten. Der Patient konnte erfolgreich operiert werden.

Wir hoffen, daß durch frühere und häufigere Anwendung der selektiven Oberbaucharteriographie der Prozentsatz ungeklärter Gastrointestinalblutungen herabgesetzt werden kann.

Summary

An aneurysm of the gastro-duodenal artery was demonstrated pre-operatively by selective arteriography in a thirty-three old patient who had suffered from continuous gastro-intestinal bleeding for six months, of sufficient severity to threaten life. All other radiological investigations, including hypotonic duodenography and aortography proved useless. A successful operation was subsequently carried out.

We hope that earlier and more frequent use of selective abdominal arteriography will reduce the number of undiagnosed cases of gastro-intestinal bleeding.

Résumé

L'angiographie sélective préopératoire permit pour la première fois de mettre en évidence un anévrisme de l'artère gastro-duodénale chez un malade de trentetrois ans, qui souffrait depuis six mois d'hémorragies intestinales continuelles inexpliquées et mettant sa vie en danger. Tous les autres examens radiologiques y compris la duodénographie sous hypotension, même l'aortographie sériée par étages restèrent sans résultat. Le malade fut opéré avec succès.

Les auteurs espèrent que l'utilisation plus précoce et plus fréquente de l'artériographie sélective de l'abdomen supérieur permettra d'abaisser le pourcentage des hémorragies gastro-intestinales inexpliquées.

Resumen

En un paciente de 33 años, aquejado desde hacía 1/2 año de gravísimas y extrañas hemorragias intestinales continuas, se pudo diagnosticar, por primera vez, por angiografía selectiva pre-operatoria, un aneurisma de la arteria gastro-duodenal. Todos los demás métodos radiográficos fracasaron, incluso la duodenografía hipotensa y la aortografía en series de capas. El paciente pudo ser operado con pleno éxito.

Esperamos que por aplicación precoz y frecuente de la arteriografía selectiva del epigastrio, pueda disminuirse el porcentaje de las hemorragias gastro-in-testinales no aclaradas.
