Ultraschallbefunde beim stumpfen Nierentrauma

Zusammenfassung

Aufgrund der günstigen Lage, der typischen Form und guten akustischen Eigenschaften sind die Nieren für die Ultraschalldiagnostik ideal geeignet. Dies gilt für jene pathologischen Veränderungen, die zu einer makromorphologischen Veränderung der Nieren führen und somit auch für das stumpfe Nierentrauma. Erstaunlicherweise finden sich aber in der Literatur kaum Erfahrungsberichte über sonographische Befunde beim stumpfen Nierentrauma.

Das eigene Krankengut bei dieser Fragestellung umfaßt 17 Patienten. Davon fanden sich in 10 Fällen pathologische Befunde. Die sonographische Treffsicherheit beim schweren Nierentrauma kann mit 100% bezeichnet werden, wenn man davon absieht, daß gelegentlich eine Hydronephrose nicht von einer größeren parapelvinen Zyste abgegrenzt werden kann.

Das Urogramm bzw. Uronephrotomogramm bildet die diagnostische Grundlage (Ausnahme: Kontrastmittelallergie und Gravidität). Die diagnostische Untersuchungsreihenfolge wird abschließend einer neuen Wertung unterzogen.

The kidneys are beautiful organs for ultrasonography, because of their position, well defined characteristic appearance and excellent acoustic qualities.

This is highly important for assessing all pathologies leading to macromorphological changes of the kidneys including these in blunt renal trauma.

We could'nt find any paper on this topic in the recent literature available for us.

Our own material in renal blunt trauma includes 17 patients. There were no pathological findings in 7, and pathological findings in 10 of them. Sonographic accuracy in severe trauma of the kidney is about 100%, excluding hydronephrotic kidneys, which cannot always be distinguished from a bigger parapelvic cyst.

Urogramm resp. Uronephrotomogramm should be performed and studied thoroughly. Exceptions are Iodine allergy and pregnancy.
