Spinale Reizerscheinungen nach Myelographien mit Amipaque bei Patienten mit schweren Kyphoskoliosen

Zusammenfassung

Bei 2 Patienten, die schwere Kyphoskoliosen aufwiesen und mit dem neuen nicht-ionisierenden Kontrastmittel Amipaque myelographiert worden waren, traten erhebliche spinale Reizerscheinungen auf. Es wird diskutiert, ob die Fehlhaltung und eine dadurch bedingte stark verlangsamte Durchflußgeschwindigkeit mit verlängerter Einwirkung auf das (vorgeschädigte?) Rückenmark dafür verantwortlich zu machen ist. Patienten mit derartigen Fehlhaltungen der Wirbelsäule bedürfen auch bei Verwendung von Amipaque einer besonders strengen Indikation zur Vornahme der Myelographie.

Summary

Two patients with severe kyphoscoliosis developed marked spinal irritation following myelography with the new non-ionising contrast medium Amipaque. The authors raise the possibility that the spinal deformity may result in reduced flow of cerebro-spinal fluid; this may prolong the contact of the contrast medium with the (previously damaged?) spinal cord. Patients with this type of spinal deformity require particularly stringent indications for the performance of myelography, even when using Amipaque.
