Die nekrotisierende Fasziitis an der Hand – Ein seltenes, hochdramatisches Krankheitsbild

Zusammenfassung

Die nekrotisierende Fasziitis (NF) ist ein in der Handchirurgie seltenes Krankheitsbild, welches durch einen foudroyanten Krankheitsverlauf gekennzeichnet ist. Pathogenetisch ist für die Entstehung der NF meist ein polymikrobielles Keimspektrum verantwortlich. Aufgrund der vitalen Bedrohung der Erkrankten muss bereits bei klinischem Verdacht auf eine NF unverzüglich eine lokal-chirurgische und systemische Behandlung erfolgen. Wir berichten über zwei Fälle einer NF an der Hand, in denen durch radikale Débridements, intensivmedizinische Maßnahmen und eine gezielte Antibiose das Leben der Patienten gerettet werden konnte.

Abstract

The necrotising fasciitis (NF) is a rare syndrome in hand surgery which is characterizsed by a severe course of the illness. Different kinds of bacterial species are responsible for the pathogenetic origin of the NF. As NF can be life-threatening for the patient, immediate surgical and systemic treatment must begin as soon as there is the slightest clinical suspicion. We report about two patients with NF of the hand, whereby the patients’ lives were saved by radical, serial débridements, intensive medical therapy and selective antibiotic treatment.
