Bildgebung der Kopfspeicheldrüsen

Zusammenfassung

Dieser Artikel stellt die häufigsten entzündlichen und tumorösen Erkrankungen der Kopfspeicheldrüsen sowie deren Charakteristika und Differenzierung in der Bildgebung vor. Es werden die heute gängigen Bildgebungsmodalitäten Sonografie, CT und MRT, die Untersuchungstechniken und Protokolle, die in der radiologischen Diagnostik dieser Erkrankungen relevant sind, besprochen. Außerdem wird die Anatomie der Speicheldrüsen und wichtiger angrenzender Strukturen, die für die Befundung und Durchführung der Untersuchungen erforderlich ist, dargestellt.

Abstract

This article presents the most common inflammatory and tumorous conditions of the salivary glands of the head and neck region as well as their imaging features and differentiation criteria. Current imaging techniques as Ultrasound, Computed Tomography and Magnetic Resonance Imaging with according protocols are discussed. Furthermore the imaging anatomy relevant for performance and assessment of imaging studies is illustrated.

Speicheldrüsen

Zu den Kopfspeicheldrüsen gehören die Glandula parotis (größte, rein seröse Speicheldrüse, Mündung über den Stenon-Gang gegenüber des 2. Molaren des Oberkiefers in die Mundhöhle), die Glandula submandibularis (zweitgrößte, seromuköse Drüse, Mündung über den Wharton-Gang lateral des Zungenbändchens auf der Caruncula sublingualis), die Glandula sublingualis (mehrere kleine muköse Einzeldrüsen mit zahlreichen kleinen Ausführungsgängen) und viele kleine, in der Submukosa liegende Speicheldrüsen.

Untersuchungsverfahren

Die konventionelle Sialografie ist lange Zeit der Goldstandard in der Bildgebung des Gangsystems der Speicheldrüsen gewesen und wird heute zunehmend von der MR-Sialografie ersetzt. Zur Darstellung des Drüsenparenchyms werden Sonografie, CT und MRT eingesetzt.

Nicht tumoröse Erkrankungen der Speicheldrüsen

• Eine Sialadenitis kann infektiös (Viren, Bakterien), radiogen oder durch Steinbildungen (besonders in der Glandula submandibularis) bedingt sein. Zystische Läsionen der Speicheldrüsen sind oft durch subtotale chronische Obstruktionen der Ausführungsgänge bedingt (meist in der Glandula submandibularis). Der Küttner-Tumor ist eine Sklerosierung der Glandula submandibularis, das primäre Sjögren-Syndrom eine Autoimmunerkrankung, die die Speicheldrüsen und die Tränendrüse betrifft. Bei HIV-Infektionen kann die Glandula parotis durch Ausbildung von benignen lymphoepithelialen Läsionen beteiligt sein.

• Der Nachweis von Abszessen und von Konkrementen ist von unmittelbarer therapeutischer (chirurgischer) Konsequenz.

Tumoren der Speicheldrüsen

• Die Speicheldrüsentumoren machen weniger als 3 % aller Kopf-Hals-Tumoren aus und weniger als 0,3 % aller Neoplasien. Häufigster Tumor ist das benigne pleomorphe Adenom, zweithäufigster benigner Tumor ist der Warthin-Tumor. Bei den malignen Tumoren ist das Mukoepidermoidkarzinom in der Glandula parotis am häufigsten, in den Glandulae submandibularis und sublingualis das adenoidzystische Karzinom.

• Ziel der Bildgebung ist es, die Lokalisation und Ausdehnung des Tumors zu bestimmen, Kriterien für benignes oder malignes Wachstum zu erkennen, das Staging durchzuführen und ggf. eine Punktion zu ermöglichen. Maligne Speicheldrüsentumoren sind anhand der Bildgebung nicht sicher voneinander zu unterscheiden.
