Myoepithelial carcinoma is a very rare epithelial carcinoma occurring usually in the salivary gland. Only about 45 cases have been reported in detail [1]. The lacrimal gland is another, but much rarer, possible tumor site, with only 2 cases reported. Because of its rarity, the clinical behavior is not well understood yet and treatment remains challenging. We describe a case of malignant transformation of a pleomorphic adenoma into a myoepithelial carcinoma.
