Abhängig von dem vorliegenden Erkrankungsstadium leiden 50–100 % der Krebspatienten an Tumorschmerzen. Neben den kontinuierlichen, gleichförmigen oder belastungs- bzw. bewegungsinduzierten Tumorschmerzen treten bei etwa 60 % der Betroffenen akute vorübergehende Schmerzspitzen auf. Wenn diese nicht auf eine unzureichende Basisschmerztherapie zurückzuführen sind und auch ein Tumorprogress oder ein sogenannter Crescendoschmerz auszuschließen sind, benötigen diese Patienten unbedingt eine zusätzliche Bedarfsanalgesie, welche diese Durchbruchschmerzen adäquat kontrolliert. Vorzugsweise wird dabei für die Basis- und die Bedarfsmedikation die gleiche Substanz eingesetzt, wobei die Dosierung der Bedarfsmedikation in der Regel 15–20 % der Tagesdosis der Basismedikation entspricht. Je schneller die Wirkung dieser zusätzlichen Analgesie einsetzt, desto gezielter kann sie im Einzelfall therapeutisch wirken. Am schnellsten wirken intravenös appliziertes Morphin (z. B. mit patientenkontrollierter Analgesie, PCA) oder orales transmukosales Fentanyl.
50–100 % of cancer patients suffer from tumour pain, depending upon the stage of the disease. Besides the continuous steady, strain-induced or movement-induced tumour pain, 60 % also suffer from acute temporary peaks of acute pain. If such pain is not due to insufficient basic therapy or to so-called „crescendo“ pain, these patients imperatively require additional analgesia that adequately controls the breakthrough pain. Preferably the same substance is employed for the basic and additional therapy; usually the additional therapy amounts to 15–20 % of the daily dosage. The more rapidly this additional analgesia acts, the more on-target therapy can be achieved in individual cases. A most rapidly acting effect is achieved by intravenously applied morphine (PCA = patient-controlled analgesia) or by oral transmucosal fentanyle.
