Zusammenfassung

Das postoperative Delir stellt sowohl im Aufwachraum als auch auf der Intensivstation die häufigste psychiatrische Erkrankung dar. Insbesondere bei beatmeten Patienten sind Prävalenzraten von über 80% beschrieben. Patienten die während ihrer Behandlung im Krankenhaus an einem Delir erkranken, haben ein 3-fach erhöhtes Risiko, in den folgenden 6 Monaten zu versterben. Ohne validierte Untersuchungsmethoden wird das Delir in den meisten Fällen nicht erkannt. Die Detektion eines Delirs ist in diesem Falle jedoch unabdingbare Voraussetzung für die frühzeitige adäquate medizinische Behandlung des Patienten. Der folgende Artikel legt seinen Schwerpunkt auf das Delir bei kritisch kranken Patienten und gibt einen Überblick über wichtige zur Verfügung stehende Messinstrumente des Delirs. Hierbei werden vor allem auch inhaltliche Unterschiede zwischen verschiedenen Scores herausgearbeitet, die bei der Auswahl eines geeigneten Messinstruments für die klinische Routine helfen können. Alle in diesem Artikel vorgestellten Delirscores liegen in einer vom Originalautor freigegebenen deutschen Übersetzung vor.

Abstract

The reported incidence of delirium in critically ill patients ranges widely – from 11% to 87%. Both in the recovery room as well as in the intensive care unit postoperative delirium is the most common psychiatric disease. Patients with ICU delirium have a significant higher 6-month mortality rate. Recent studies could show that the use of a validated delirium assessment tool significantly improves the ability of physicians and nurses to detect delirium in ICU patients. The following article gives a review about different assessment tools of ICU delirium and focuses on the differences between validated delirium scores.

• Die 4 Hauptmerkmale des Delirs nach DSM-IV sind: Bewusstseinsstörung, Veränderung der kognitiven Funktion, Entwicklung des Störungsbildes innerhalb von Stunden oder Tagen sowie die Feststellung eines medizinischen Krankheitsfaktors.

• Die DSM-IV-Kriterien als „Goldstandard“ zur Validierung von Screeninginstrumenten für das Delir sind besonders gut geeignet, weil diese die Mehrzahl an gefährdeten Patienten mit Delirsymptomatik und einem signifikant schlechteren Outcome definieren.

• Es gibt 3 Formen von postoperativen kognitiven Störungen: das Emergence Delirium, das postoperative Delir und die postoperative kognitive Dysfunktion (POCD). Inwieweit diese Störungsbilder sich gegenseitig bedingen oder ob sie unabhängig voneinander auftreten können, ist bislang nicht geklärt.

• Nach den Empfehlungen der S2e-Leitlinie sollte bei jedem Patienten auf Intensivstationen alle 8 Stunden ein Delirscoring durchgeführt werden.

• Zur Delirdiagnostik sollten standardisierte Messverfahren angewendet werden. Delirscores sollten in einer „richtlinienkonformen Übersetzung“ vorliegen und zuvor verblindet gegen den „Goldstandard“ getestet worden sein. Des Weiteren ist es wichtig, den für die entsprechende Situation geeigneten Test zu finden und zu nutzen.

• Mithilfe der CAM-ICU kann man das Delir bei Intensivpatienten anhand von 4 Items bestimmen. Für den klinischen Alltag gibt es ein anschauliches „Flowsheet“.

• Die ICDSC ist eine Skala mit 8 Items, die jeweils mit 0 Punkten oder 1 Punkt bewertet werden. Ein Delir liegt vor, wenn der Patient mindestens 4 Punkte hat. Patienten mit einem ICDSC von 1–3 Punkten (subsydromales Delir) erfüllen zwar nicht die Kriterien eines voll ausgeprägten Delirs, haben aber ein schlechteres Outcome als Patienten ohne Delir (0 Punkte).

• Die Nursing Delirium Screening Scale (Nu-DESC) ist ein pflegebasiertes Instrument zur Delirdiagnostik. In einer Validierungsstudie konnte eine hohe Validität demonstriert werden. Dabei betrug die mittlere Durchführungszeit nicht länger als 1 Minute pro Patient.

• Die Kernaussagen aller Studien weisen immer wieder darauf hin, dass die entscheidende Maßnahme zur Verbesserung der medizinischen Versorgung das systematische Delirscreening ist – zunächst unabhängig davon, welcher der Scores letztendlich Verwendung findet.
