Postinflammatorische Gehörgangsstenose

Zusammenfassung

Die postinflammatorische Gehörgangsstenose (Synonym: stenosierende Otitis externa, postinflammatorische mediale meatale Fibrose-PIMMF) und ihr klinisches Erscheinungsbild, ihre Ursachen und ihre Therapie werden in der Weltliteratur nur selten abgehandelt. Diese Pathologie scheint aufgrund chronischer Entzündungsprozesse von Gehörgang, Trommelfell und/oder Tympanon und deren Abheilung mit Ersatz des ortsständigen Epithels durch neugebildetes Bindegewebe zu entstehen und kann sich bis zur vollständigen Obliteration des medialen Gehörgangs entwickeln. Die lateralen Gehörgangsabschnitte bleiben dabei typischerweise frei und imponieren nach medial als blind endender Sack. Die Patienten zeigen eine je nach Befund entsprechende bis stark ausgeprägte Schallleitungsschwerhörigkeit. Die Therapie besteht in der ersten entzündlichen Phase durch Lokaltherapie, in der bindegewebigen Phase dann durch die Operation. Hierbei wird das Bindegewebe vollständig reseziert und die Defekte durch Hauttransplantate gedeckt. Durch diese Technik lassen sich dauerhaft rezidivfreie, weite Ohren mit gutem Hörvermögen wiederherstellen.

Abstract

There are only few publications about acquired postinflammatory stenosis of the auditory canal, its etiology, its progress and an adequate therapy. Chronic inflammatory processes of the ear canal, the tympanic membrane or the middle ear could lead to a replacement of the origin epithelium by fibrotic tissue and ends in a total obliteration of the ear canal. Lateral parts of the canal remain open and have the form of a blind-ending sac. Patients can suffer from severe conductive hearing loss. Treatment implies local therapy in the first stadium of the disease, then in the second one surgical removement of the fibrotic tissue is indicated before ear drum and bony canal are covered with skin graft. This technique results in stable, wide ears with good hearing.
