Arthrodesen des Lisfranc- und Chopart-Gelenks bei Synovialkrankheiten

Zusammenfassung

Die unteren Sprunggelenke und die Mittelfußgelenke können bei entzündlichen Synovialkrankheiten einzeln oder gemeinsam destruiert und schmerzhaft sein. Ursächlich hierfür sind Gelenkzerstörungen, Deformitäten, Instabilitäten und Gelenksteifen durch Sehnenrupturen, Knorpel- und Knochenverluste und neuropathische Veränderungen mit gestörter Tiefensensibilität. Prinzipiell werden nur solche Gelenke versteift, die Beschwerden verursachen oder die als Teilursache schmerzhafter Fußdeformitäten identifiziert werden. Die Beweglichkeit umgebender Gelenke soll nach Möglichkeit erhalten bleiben, um Anschlussarthrosen zu verhindern. Schraubenosteosynthesen scheinen gegenüber der Verwendung von Staples oder Klammern den Vorteil einer geringeren Pseudarthroserate zu haben.

Abstract

Pain in or symptomatic destruction of any number of the joints of the midfoot can have several causes. Besides muscular and/or tenoligamentous instability, late stages of various inflammatory arthritides (rheumatoid arthritis, seronegative spondarthritis, collagenoses), and neuropathic diseases with impaired depth perception (cervical myelopathy of the foot in advanced rheumatoid arthritis) can cause disorders of the Lisfranc and Chopart joints. As a general policy, only joints that are causing symptoms, are clearly arthritic, or are involved in a fixed foot deformity should be fused, so that as much of the residual midfoot motion as possible is preserved. Thishelps to restrict future osteoarthritis in adjacent joints. The use of screws rather than staples seems to increase the fusion rates because this technique allows compression on the fusion.
