Zerebrale Mikroangiopathien

Zusammenfassung

Die zerebralen Mikroangiopathien sind eine heterogene Krankheitsgruppe, bei der sich als gemeinsames Charakteristikum pathologische Veränderungen der kleinen Hirngefäße finden. Sie sind für 20 – 30 % aller ischämischer Schlaganfälle verantwortlich. In über 90 % der Fälle liegt eine erworbene Mikroangiopathie vor, wobei die degenerative Mikroangiopathie und die sporadische zerebrale Amyloidangiopathie die typischen erworbenen Mikroangiopathien im engeren Sinne darstellen. Neben diesen häufigen Formen gibt es eine Vielzahl seltener, genetisch bedingter Mikroangiopathien. Wichtige hereditäre Mikroangiopathien sind z. B. die CADASIL-Erkrankung (Cerebral Autosomal Dominant Arteriopathy with Subcortical Infarcts and Leukoencephalopathy), der Morbus Fabry und das MELAS-Syndrom (Mitochondriale Enzephalopathie mit Laktatazidose und Schlaganfällen).

Abstract

Cerebral microangiopathies are a very heterogenous group of diseases characterized by pathological changes of the small cerebral vessels. They account for 20 – 30 % of all ischemic strokes. Degenerative microangiopathy and sporadic cerebral amyloid angiography represent the typical acquired cerebral microangiopathies, which are found in over 90 % of cases. Besides, a wide variety of rare, hereditary microangiopathy exists, as e. g. CADASIL (Cerebral Autosomal Dominant Arteriopathy with Subcortical Infarcts and Leukoencephalopathy), Fabrys disease and MELAS syndrome (Mitochondrial myopathy, Encephalopathy, Lactic Acidosis, and Stroke-like episodes).

Bei den zerebralen Mikroangiopathien handelt es sich um eine sehr heterogene Krankheitsgruppe. Als gemeinsames Charakteristikum finden sich pathologische Veränderungen der kleinen Hirngefäße. Die häufigen sporadischen Formen der Mikroangiopathien müssen von den seltenen hereditären Mikroangiopathien unterschieden werden. Die degenerative Mikroangiopathie und die sporadische zerebrale Amyloidangiopathie sind die typischen erworbenen Mikroangiopathien.

Die genauen Pathomechanismen, die den Parenchymveränderungen bei der degenerativen Mikroangiopathie zugrunde liegen, sind noch nicht im Einzelnen geklärt. Typische mikroangiopathische Parenchymläsionen wie lakunäre Infarkte und Marklagerläsionen („Leukoaraiose”) sollten nur im klinischen Kontext gewertet werden. Wichtige hereditäre Mikroangiopathien sind die CADASIL-Erkrankung, der Morbus Fabry und das MELAS-Syndrom
