Epidemiologie und Einteilung

S. aureus ist der zweithäufigste Erreger nosokomialer Bakteriämien. Die nosokomiale S. aureus-Bakteriämie (SAB) macht zwischen 40 – 60 % aller SAB aus.

Noskomiale und Gesundheitssystem-assoziierte SAB unterscheiden sich hinsichtlich Komorbidität der Patienten, Eintrittspforte der Infektion, MRSA-Rate und Komplikationen von der ambulant erworbenen SAB.
Eintrittspforte, Komplikationen
-
Intravaskuläre periphere oder zentrale Katheter stellen die mit Abstand häufigste Eintrittspforte bei der nosokomialen SAB dar.

Wichtige Komplikationen der SAB umfassen tiefsitzende metastatische Absiedlungen, Osteomyelitis, Spondylodiszitis, Endokarditis und schwere Sepsis/septischer Schock.

Sorgfältige Anamnese, klinische Untersuchung sowie ausführliche Diagnostik zur Evaluation der Eintrittspforte, des Schweregrades und der Ausbreitung der Infektion sind von größter Bedeutung.
Management und Therapie

Die Therapie der Wahl bei MSSA-SAB stellt in Deutschland Flucloxacillin (4 × 2 g/d i. v. bei normwertiger Nierenfunktion) dar. Zur Behandlung der MRSA-SAB kommen in erster Linie Vancomycin (15 mg/kg KG alle 12 h i. v., Ziel-Talspiegel 10 – 15 µg/ml), Daptomycin (6 mg/kg KG/d i. v.) oder Linezolid (2 × 600 mg/d i. v.) infrage.

Für die Therapiedauer ist entscheidend, ob eine komplizierte oder unkomplizierte SAB vorliegt. Bei einer unkomplizierten SAB wird bislang eine Therapiedauer von 2 Wochen empfohlen. Die komplizierte SAB bedarf einer mindestens 4-wöchigen antibiotischen Therapie.

Fortgeschrittenes Alter, schwere Komorbidität, Endokarditis, septischer Schock, Aufnahme auf die Intensivstation, persistierende Bakteriämie und unklare Eintrittspforte stellen unabhängige Risikofaktoren für ein schlechteres Outcome dar.

Als „Quality of Care“-Indikatoren können die Abnahme von Kontroll-Blutkulturen, die frühzeitige Entfernung infizierter Katheter und die Durchführung einer transösophagealen Echokardiografie angesehen werden. Durch Einbeziehung eines infektiologischen Konsiliardienstes kann die Adhärenz an diese Qualitätsindikatoren sowie an diagnostische und therapeutische Standards gesichert und damit die Prognose (Krankenhaus- oder 30-Tage-Sterblichkeit) der SAB verbessert werden.
Prävention

Insbesondere die nosokomiale SAB (als auch die Gesundheitssystem-assoziierte SAB) kommen als Ziel von Präventionsmaßnahmen und Infektions-Kontrollprogrammen infrage. Sinnvolle Strategien umfassen multimodale Programme (theoretische Fortbildungen in Kombination mit praktischen Instruktionen und Übungen) sowie die Implementierung von „Catheter Bundles“. Eine topische S. aureus-Dekolonisierung von Nase und Haut mit Mupirocin bzw. Chlorhexidin scheint nach neueren Studienergebnissen die Rate nosokomialer S. aureus-Wundinfektionen reduzieren zu können. Inwieweit hierdurch auch die Rate der nosokomialen SAB beeinflusst werden kann, ist noch unklar.
