Zusammenfassung
Der Einsatz von Ultraschall hat im Bereich der Intensivmedizin große Fortschritte gemacht und ist aus der bettseitigen Diagnostik des Intensivpatienten kaum mehr wegzudenken. Die Vorzüge der Sonografie liegen in der geringen Komplikationsrate, in der schnellen Verfügbarkeit und in einer hohen diagnostischen Aussagekraft gerade in lebensbedrohlichen Situationen. Dieser Beitrag beschreibt den Einsatz der Sonografie auf der Intensivstation mit Blick auf die Bereiche Echokardiografie, transkranielle Doppler-Sonografie, Abdomen-Sonografie und Sonografie von Lunge und Pleura. Darüber hinaus wird auf ultraschallgesteuerte Punktionen eingegangen.
Abstract
Ultrasonography has developed as an indispensable and highly versatil tool in the diagnosis and treatment of critically ill patients. Nowadays intensive care medicine without the use of ultrasound is hard to imagine. The advantages of ultrasound are instantaneous availability at the bedside and high diagnostic output especially in life-threatening situations. This article focuses on special applications of ultrasound in the intensive care unit: echocardiography, transcranial Doppler sonography, abdominal ultrasonography and the imaging of pleura and lung. Furthermore, the use of ultrasound to guide interventions is discussed.

Die Sonografie ist der konventionellen Röntgen-Thoraxaufnahme bei der Diagnose einer großen Mantelpneumothorax gleichwertig, hat jedoch zusätzlich eine wesentlich höhere Sensitivität im Nachweis des okkulten ventralen Pneumothorax.

Bei stumpfen Bauchtraumata bietet die Abdomensonografie den besonderen Vorteil, bettseitig verfügbar zu sein und kurzfristig Verlaufskontrollen zum Ausschluss einer zweizeitigen Organruptur vornehmen zu können.

Mithilfe der Sonografie kann auch beim sedierten Patienten eine peritoneale Symptomatik differenziert werden.

Der Ultraschalleinsatz erleichtert die Punktion eines Pleuraergusses sowie die Einlage einer Drainage.
