Aktuelle klinische Untersuchung der Schulter

Zusammenfassung

In den letzten Jahren haben wir infolge der verbesserten bildgebenden Verfahren und insbesondere der Arthroskopie Einblicke in die eigentlich ablaufenden pathologisch-anatomischen Veränderungen gewonnen, die Schulterschmerzen verursachen. Gleichzeitig entwickelte sich eine subtilere Untersuchungstechnik, die mithilfe einer zielgerichteten Anamnese die Differenzialdiagnosen auf wenige Pathologien eingeengt. Die klinische Antwort verschiedener Krankheitsbilder ist unspezifisch und äußert sich in Schmerzen, Bewegungseinschränkung, Kraftminderung und Instabilitäten. Im Gegenteil zu einem weitgehend knöchern geführten Gelenk wie dem Hüftgelenk führen am Schultergelenk insbesondere Weichtteilveränderungen zu einer klinischen Symptomatik. Daraus resultierte der Begriff „Periarthritis humeroscapularis”, der jedoch zu allgemein gefasst ist und keine spezifische Therapie erlaubt. Die klinische Untersuchung der Schulter zielt nach einem Ausschluß relevanter knöcherner Veränderungen im Röntgenbild auf Veränderungen der Gelenkkapsel, der Sehnen und Muskeln sowie der Schleimbeute. In einer standardisierten Untersuchung können das Bewegungsausmaß, die Kraftentfaltung und einzelne anatomische Strukturen mittels Palpation untersucht werden. Spezifischer Funktionstests lassen eine weitere Differenzierung zu. Dies ermöglicht die weitere Weichenstellung zur Diagnostik mittels Sonografie und Kernspintomografie, sodass zügig eine Arbeitsdiagnose gestellt werden kann.

Abstract

Knowledge about shoulder pathologies has increased tremendously over the last years, e. g., with the help of basic science, arthroscopy and improved radiological imaging. In parallel subtle examination techniques have been developed which allow for an exact differential diagnosis. This is of great importance since different pathologies present with unspecific symptoms and signs such as pain, instability, loss of range of motion and power. As the shoulder is a soft tissue-balanced joint structures such as the ligaments, capsule, tendons and muscles have to be given special attention. In a standardised examination the active and passive ranges of motion, the muscle strength and specific anatomic structures have to be tested. Furthermore, special functional tests allow a further differentiation. Based on the clinical examination and the patient's history further diagnostic tools such as sonography and/or MRI can be ordered to obtain a diagnosis.
