Bestimmung des Ernährungszustands: moderne Standards

Zusammenfassung

Eine Abschätzung des Ernährungszustands gehört zu jeder ärztlichen Untersuchung, um eine mögliche Mangelernährung rechtzeitig zu erkennen. Die Überernährung lässt sich anhand des Body-Mass-Indexes (BMI) leicht diagnostizieren und graduieren. Der BMI ist für die Diagnostik der Mangelernährung jedoch nicht geeignet. Leitsymptom der krankheitsassoziierten Mangelernährung ist der signifikante unbeabsichtigte Gewichtsverlust (z. B. > 10 % in 6 Monaten). Dieses Kriterium findet sich in allen etablierten Scores für das Screening auf Mangelernährung (z. B. NRS-2002 [Nutritional Risk Screening 2002], SGA [Subjective Global Assessment], MUST [Malnutrition Universal Screening Tool], MNA [Mini Nutritional Assessment]). Die Zusammensetzung des Körpers ist eine wesentliche Größe der klinischen Ernährungsmedizin, da jede relevante Veränderung des Ernährungszustands mit Änderungen der Körperzusammensetzung einhergeht. Diese korreliert mit Verschlechterungen von Körperfunktionen und einem ungünstigeren Krankheitsverlauf. Methoden, um die Körperzusammensetzung zu bestimmen, die für die klinische Praxis infrage kommen, sind durchweg indirekte Methoden wie die Anthropometrie oder bioelektrische Impedanzanalyse (BIA).

Abstract

The assessment of the nutritional status is an integral part of the physical examination. Overnutriton can be easily assessed by the body mass index. The key symptom of disease related malnutrition is unintended weight loss (i. e. > 10 % in 6 months). This parameter is part of all established scores recommended for screening of malnutrition (NRS-2002, SGA, MUST, MNA). The body mass index itself is of limited value although it is considered in some of the scores. The body composition analysis is an established approach of clinical nutrition because every significant change of the nutritional status is associated with changes of body composition. These changes correlate with impairment of physical function and influences clinical prognosis. All methods suitable for clinical practice are indirect methods such as anthropometry or bioelectrical impedance analysis (BIA).
