IgA-Nephropathie – Neues zur Pathogenese und Therapie

Aktuelle Befunde haben zu einem besseren Verständnis der Pathogenese der IgA-Nephropathie beigetragen: Vermindert galaktosyliertes IgA₁ ist im Serum und in den mesangialen Ablagerungen gehäuft nachweisbar. Zusätzlich zeigten sich im Serum IgG-Autoantikörper gegen vermindert galaktosyliertes IgA₁. Genetische Studien deckten einen Zusammenhang zwischen einzelnen HLA-Polymorphismen und dem Auftreten einer IgA-Nephropathie auf. Die optimale Therapie der IgA-Nephropathie bleibt weiter unklar. Der Wert einer niedrig normalen Blutdruckeinstellung bei proteinurischen IgA-Nephropathie-Patienten ist unumstritten. Die beste Evidenz bei den Immunsuppressiva besteht für Kortikosteroide bei proteinurischen IgA-Nephropathie-Patienten mit einer normalen oder gering eingeschränkten Nierenfunktion. Keinen zusätzlichen Nutzen bei gehäuften Nebenwirkungen bringt eine Kombinationstherapie aus Steroiden und Azathioprin bei diesem Patientengut. Den möglichen Nutzen einer immunsuppressiven Therapie zusätzlich zu einer bereits optimierten supportiven Therapie untersucht aktuell die deutschlandweit laufende Studie STOP IgAN.

Recent findings have improved our understanding of IgA nephropathy (IgAN) pathogenesis: Poorly galactosylated IgA₁ is increased in serum and mesangial deposits. Additionally, IgG-autoantibodies against poorly galactosylated IgA₁ are detectable in serum. Genetic studies revealed an association between certain HLA-polymorphisms and the occurence of IgAN. The optimal therapy of IgAN remains uncertain. The value of a low normal blood pressure control in proteinuric IgAN patients is not controversial. The best evidence for immunosuppressants exists for corticosteroids in proteinuric IgAN patients with normal or mildly reduced renal function. No additional benefit but more side effects are observed in a combination therapy of steroids and azathioprine in such patients. The potential benefit of an immunosuppressive therapy in addition to an already optimized supportive therapy is currently under investigation in the ongoing German STOP IgAN Study.
