Generalisiertes anuläres elastolytisches Riesenzellgranulom bei hochmalignem B-Zell-Lymphom

Zusammenfassung

Wir berichten über einen 91-jährigen Patienten mit seit über 10 Jahren bestehenden, zum Teil zentral eingesunkenen, polyzyklisch begrenzten oder deutlich randbetonten, erythematösen, mit Ausnahme des Gesichtes generalisierten Hautveränderungen. Histologisch diagnostizierten wir ein anuläres elastolytisches Riesenzellgranulom. Gleichzeitig wurde bei dem Patienten neu ein diffus-großzelliges B-Zell-Lymphom im Stadium IVA nach Ann-Arbor-Klassifikation mit Knochenmarksinfiltration, multiplem Lymphknotenbefall und Milzherden festgestellt. Unter einer Therapie mit Rituximab waren nach 5 Wochen zwar die Lymphome, nicht jedoch die Hautveränderungen deutlich rückläufig.

Abstract

We report on a 91-years old male caucasian patient with multiple partly centrally depressed polycyclic or strongly margin stressed, erythematous skin lesions, generalized with the exception of the face, since ten years existing. The diagnosis of an annular elastolytic giant cell granuloma (AEGCG) was made by histology. In parallel, a newly detected diffuse B-cell lymphoma stage IVA Ann-Arbor classification with multiple lymph node, spleen and bone marrow involvement was diagnosed. During a therapy with rituximab the lymphoma responded well within 5 weeks, but the skin lesions did not improve markedly.
