Nebenwirkungen von Antidepressiva

Kernaussagen

• Auch subjektive Nebenwirkungen ohne eindeutige biochemische Erklärung verdienen Beachtung. Sie beeinflussen die Compliance.

• Miktionsstörungen, sexuelle Funktionsstörungen und Gewichtszunahme führen besonders oft zum Absetzen.

• Vegetative Nebenwirkungen sind bei Trizyklika besonders häufig, Stürze bei SSRI.

• Fahreignung ist unter nicht sedierenden Antidepressiva in der Regel gegeben, wenn die depressive Symptomatik weitgehend remittiert ist.

• Trizyklika sind wegen der kardialen Nebenwirkungen bei schwer Herzkranken ungünstig und erhöhen das Herzinfarktrisiko.

• Bei Jugendlichen und jungen Erwachsenen können unter SSRI Suizidgedanken häufiger werden. Ein erhöhtes Risiko für Suizide ist nicht nachzuweisen.

• Das Risiko des Umschlagens einer Depression in eine Hypomanie bzw. Manie ist bei denjenigen Patienten häufiger, die eine bipolare Störung haben oder dafür prädisponiert sind.

• Absetzphänomene sind häufig, meist ungefährlich, aber belastend.

• Da teratogene Risiken nicht auszuschließen sind, sollte in der Frühschwangerschaft und bei Kinderwunsch, wenn möglich, auf Antidepressiva verzichtet werden.

• Abstillen ist bei SSRI und Nortriptylin nicht unbedingt erforderlich.
