Zusammenfassung
Im Hinblick auf die Vielzahl der zur Verfügung stehenden Plattensysteme ist es sinnvoll, ein System vorzuhalten, mit dem alle Versorgungstypen – palmare und dorsale Stabilisierung, Doppelplattenosteosynthese und Korrekturosteotomie – realisiert werden können. Multidirektionale Systeme bieten bei schwierigen Situationen Vorteile und ermöglichen die subchondrale Schraubenplatzierung, die besonders bei Trümmerfrakturen und osteoporotischer Knochenstruktur wichtig ist, um sekundäre Korrekturverluste zu vermeiden.
Fractures of the Distal Radius – Selection of Implants
The majority of distal radius fractures nowadays are fixed through a palmar approach using locking devices. In the beginning mainly unidirectional devices with limited variation of screw placement were available. Nowadays looking at the wide variety of plates on the market, polyaxial devices with two rows in the distal part represent the best solution for achieving rigid support. For biomechanical reasons subchondral placement through the first and second row should be used especially in cases of comminuted fractures and osteoporotic bone quality. This helps to avoid a secondary loss of reduction.
