es wird über den Todesfall einer pulmonal schwerst vorerkrankten Patientin (COPD-Stadium IV) berichtet, welche nach endotrachealer Intubation unter PEEP-Beatmung durch Ruptur einer Emphysemblase einen Spannungspneumothorax entwickelte. Dieser wurde unter der folgenden Reanimation nicht entlastet, die Patientin verstarb. Der dargestellte Fall verdeutlicht die Notwendigkeit eines strukturierten Vorgehens bei Patienten mit Beatmungsproblemen, insbesondere hinsichtlich der Entlastung eines möglichen Spannungspneumothorax unter Reanimation.
Abstract
We present the fatality of a female patient with pre-existing severe pulmonary disease (COPD stage IV) who developed a tension pneumothorax from rupture of an emphysematous bullous during PEEP ventilation after endotracheal intubation. The tension pneumothorax was not decompressed during subsequent resuscitation efforts and the patient died. The case illustrates the need for a structured approach to patients with ventilation problems, especially with regard to decompression of a possible tension pneumothorax during resuscitation.
