Eine Sepsis bei immunsupprimierten Patienten stellt eine besondere Herausforderung für den Intensivmediziner dar, sowohl in der Diagnostik als auch in der Therapie. Die unzureichende bzw. fehlende Immunreaktion des Patienten auf die Bakteriämie und auf die zugrundeliegende Infektion führt in der Regel zum Ausbleiben von typischen, diagnostisch wertvollen Infektionszeichen wie Infiltraten, Leukozytose oder immunserologischen Reaktionen. Auch Verlauf und Ausgang einer Sepsis werden durch ein inkompetentes Immunsystem negativ beeinflusst, demgemäß tritt das Vollbild des septischen Schocks meist schnell ein und ist mit einer sehr hohen Letalität verbunden. Hieraus resultiert für alle beteiligten Disziplinen ein hoher zeitlicher Druck in der Einleitung aller therapeutischen Maßnahmen. Meist muss eine auf Empirie basierende kalkulierte antiinfektiöse Therapie ohne hinreichend verwertbare diagnostischen Hilfen durchgeführt werden.
Sepsis in immunosuppressed patients represents a special challenge for intensive care physicians with regard to both diagnosis and therapy. The inadequate or, respectively, absent immune reaction of the patient to the bacteremia and to the underlying infection generally does not lead to the appearance of typical, diagnostically relevant signs of infection such as infiltrates, leukocytosis or immunoserological reactions. Also the course and outcome of the sepsis are negatively affected by an incompetent immune system; accordingly the full clinical picture of a septic shock usually develops rapidly and is associated with a high lethality. For this reason all participating disciplines are immediately placed under high pressure of time to initiate all necessary therapeutic measures. In many cases the anti-infectious therapy has to be based more on empirical experience in the absence of diagnostically relevant and quantifiable parameters.
