Zusammenfassung
Die medikamentöse, interventionelle und operative Therapie der koronaren Herzkrankheit konnte in den letzten Jahren stetig verbessert werden. Neue pharmakologische Ansätze, die Einführung neuer Techniken und Materialien, bspw. der Medikamentfreisetzenden Stents (DES) oder Ballons (DEB), neue Katheterverfahren und nicht zuletzt neue und schonende Operationstechniken haben die myokardiale Revaskularisierung optimiert. Trotz dieser z. T. revolutionären Ansätze bleiben Patienten übrig, bei denen es nicht gelingt, die pektanginöse Beschwerdesymptomatik effektiv zu behandeln. Ein alternativer Therapieansatz bei der refraktären Angina ist die Neurostimulation. Hierbei handelt es sich um einen sicheren und effektiven Ansatz, der zu einer Linderung der Beschwerden führt und die körperliche Leistungsfähigkeit verbessert, ohne jedoch die Warnsymptome eines Myokardinfarkts zu verschleiern.
Abstract
During the last years the different therapies (pharmacological, interventional, surgical) in patients with coronary artery disease have been improved. Novel pharmacological approaches, new stent and balloon technologies and optimized interventional besides surgical techniques have improved myocardial revascularization. Despite great advances, there are still patients who suffer from refractory angina. An alternative therapeutic approach for these patients is neuromodulation. This is a safe and effective approach, which ameliorates angina and improves physical performance capacity without masking alarm signals of a myocardial infarction.
