Katheterablation bei paroxysmalem Vorhofflimmern – bereits vor Beginn einer medikamentösen antiarrhythmischen Therapie?

Abstract
Atrial fibrillation (AF) is the most common arrhythmia especially in elderly persons. Pulmonary vein isolation has become an accepted treatment option in symptomatic patients with paroxysmal and persistent atrial fibrillation after failed antiarrhythmic medication. Today two studies are available, which examined the first line pulmonary vein isolation in young patients with paroxysmal atrial fibrillation and no significant structural heart disease compared to antiarrhythmic medication and could show an advantage for the ablation strategy. Therefore in patients with paroxysmal atrial fibrillation and no significant structural heart disease pulmonary vein isolation can be recommended as first line therapy.
