In the past, MOG antibodies have been thought to be associated with the pathological events leading to MS. Several studies in large cohorts found only rarely children who were finally diagnosed with MS as having MOG antibodies at initial presentation, pleading strongly against a MS diagnosis in children with acquired demyelinating syndromes (ADS) and positive MOG antibodies. On the other hand, serum MOG antibodies have been detected in several recurrent non-MS demyelinating inflammatory diseases such as ADEM followed by optic neuritis (ADEMON), AQP4-Ab-negative neuromyelitis spectrum disorders (NMOSD), recurrent optic neuritis and more recently in children with multiphasic disseminated encephalomyelitis (MDEM). This presentation will summarize important clinical and neuroradiological aspects of children with ADS and MOG antibodies.
