Summary
Aim: Despite great advances in ultrasound diagnosis of the venous system, there is still a need for a quick and simple method of complex preoperative visualisation of the superficial venous system and its incompetent perforators. The aim of the study was to assess the usefullness of thermography in the preoperative mapping of the superficial venous system among patients with primary and recurrent varicose veins. Method: Authors examined prospectively 48 patients with varicose veins (36 with primary and 12 with recurrent varicose veins). Preoperative assessment was performed by means of infrared scanning (thermography). The results were verified by colour coded duplex ultrasound and intraoperative findings. In the results, the mapping of incompetent veins by thermography unit showed an excellent correlation with colour/duplex scan and intraoperative findings. The sensitivity and specificity for thermography were 85.4% and 93.7% respectively and were lower than the values acquired by US scanning (sensitivity and specificity – 89.6% and 97.7% respectively). Conclusion: Authors conclude, that colour/duplex ultrasound remains a method of choice in the preoperative evaluation of the entire venous system, but thermography offers a simple and quick method for complex mapping of the incompetent superficial venous system. This method might prove to be a helpful adjunct to the ultrasound, particularly in case of recurrent varicose veins or in cases, when the incompetent veins are not well seen (e. g. obesity).
Zusammenfassung
Ziel: Trotz großer Fortschritte bei der Ultraschalldiagnostik des Venensystems besteht nach wie vor Bedarf an einer raschen und einfachen Methode der komplexen präoperativen Darstellung des oberflächlichen Venensystems und dessen insuffizienter Perforansvenen. In der Studie sollte der Nutzen der Thermographie beim präoperativen Mapping des oberflächlichen Venensystems bei Patienten mit primären und rezidivierenden Varizen untersucht werden. Methode: Die Autoren führten bei 48 Patienten mit Varizen (36 mit primären und 12 mit rezidivierenden Varizen) eine prospektive Untersuchung durch. Die präoperative Untersuchung erfolgte mittels Infrarot-Scanning (Thermographie). Die Ergebnisse wurden anhand einer farbkodierten Duplexsonographie und der intraoperativen Befunde verifiziert. Ergebnis: Das Mapping der insuffizienten Venen mittels Thermographie zeigte eine ausgezeichnete Korrelation mit der Farbduplexsonographie und den intraoperativen Befunden. Für Sensitivität und Spezifität der Thermographie ergaben sich Werte von 85,4% bzw. 93,7%; sie lagen damit niedriger als die Werte, die man für die Ultraschalluntersuchung erhielt (Sensitivität: 89,6%; Spezifität: 97,7%). Schlußfolgerung: Die Farbduplexsonographie bleibt die Methode der Wahl bei der präoperativen Untersuchung des gesamten Venensystems, die Thermographie bietet jedoch eine einfache und rasche Methode für ein komplexes Mapping des insuffizienten oberflächlichen Venensystems. Diese Methode könnte sich als hilfreiche Zusatzdiagnostik zur Ultraschalluntersuchung erweisen, vor allem bei Venenrezidiven oder in Fällen, in denen die insuffizienten Venen nicht gut erkennbar sind (z. B. bei Adipositas).
Résumé
Objet: Malgré les grands progrès réalisés par les ultrasons dans le diagnostic du système veineux, la nécessité d’une méthode rapide et simple de visualisation préopératoire complexe du système veineux superficiel et des perforants incompétents existe encore toujours. Le but de l’étude était d’évaluer l’utilité de la thermographie dans la cartographie préopératoire du système veineux superficiel chez les patients atteints de varices primaires et récurrentes. Méthode: A titre prospectif, les auteurs ont examiné 48 patients atteints de varices (36 atteints de varices primaires et 12 atteints de varices récurrentes). L’évaluation préopératoire a été réalisée au moyen du balayage infrarouge (thermographie). Les résultats ont été contrôlés par balayage duplex à codage couleur et constatations intraopératoires. En ce qui concerne les résultats, la cartographie des veines incompétentes réalisée par thermographie montrait une excellente corrélation avec le balayage duplex à codage couleur et les constatations intraopératoires. La sensibilité et la spécificité pour la thermographie étaient de 85,4% et 93,7% et plus basses que les valeurs obtenues par balayage US (sensibilité et spécificité de 89,6% et 97,7%). Conclusion: Les auteurs en ont conclu que le balayage duplex à codage couleur reste la méthode de choix pour l’évaluation préopératoire du système veineux complet, mais que la thermographie constitue une méthode simple et rapide pour la cartographie complexe du système veineux superficiel incompétent. Cette méthode peut s’avérer être un adjuvant précieux des US, particulièrement dans le cas de varices récurrentes ou dans les cas où les veines incompétentes ne sont pas très visibles (p.ex. en cas d’obésité).
