Zusammenfassung

Impfungen bei Kindern mit allergischen Erkrankungen sind mit Unsicherheiten verbunden. Oftmals wird aus Angst vor schwerwiegenden Reaktionen nicht geimpft. Grundsätzlich können sehr selten Antigene, Hilfs- und Konservierungsstoffe sowie Reste von Kulturmedien für eine allergische Reaktion verantwortlich sein. Daher sollten immer Vorkehrungen getroffen werden, einen allergischen Zwischenfall nach der Immunisierung zu behandeln. Für Kinder mit einer bekannten Allergie, beispielsweise der Hühnereiweißallergie (HEA), wurde häufig angenommen, dass diese allergisch reagierten, wenn sie mit einem Impfstoff immunisiert wurden, der Spuren von Hühnereiweiß enthielt (z.B. MMR, Influenza).

Es kann mittlerweile jedoch als gesichert gelten, dass Kinder mit einer HEA kein erhöhtes Anaphylaxierisiko nach der MMR-Impfung haben. Eine Hauttestung vor der MMR-Impfung oder fraktionierte Gaben sind daher unnötig. Bezüglich der Influenza-Impfung muss die Hauttestung vor der Impfung empfohlen werden. Fraktionierte Gaben oder eine Chemoprophylaxe sind zu erwägen.

Summary

Patients with allergy especially those with concomitant egg allergy might be denied active immunization because of the risk of inducing adverse reactions with vaccines (MMR, Influenza) derived from egg embryo tissue. Despite the previous discussion the MMR vaccine is as safe as any other vaccine and children with an allergy to eggs must not have their vaccinations delayed. Skin testing for reactions to the MMR vaccine lacks specificity and sensitivity in predicting a serious allergic response and desensitation is a procedure that lacks a scientific rationale.

Evidence supports the relatively safe administration of influenza vaccine to individuals with egg allergy.
