Zusammenfassung
In der Differenzialdiagnostik entzündlich rheumatischer Systemerkrankungen müssen Malignome stets berücksichtigt werden. Anhand der fünf vorgestellten Fällen wird demonstriert, wie trotz umfassender Vordiagnostik ein Malignom zunächst nicht erkannt und fälschlich eine rheumatologische Diagnose gestellt wird. Die Vordiagnosen Sklerodermie, Sjögren-Syndrom mit Neuropathie, Polymyalgia rheumatika und rheumatische Serositis wurden widerlegt zu Gunsten eines kleinzelligen Bronchialkarzinoms, eines Uteruskarzinoms, eines Morbus Hodgkin, eines Mesothelioms und eines malignen fibrösen Histiozytoms.
Summary
The differential diagnosis of inflammatory rheumatic diseases has to consider malignancies. The following five case reports demonstrate the false diagnosis of rheumatic diseases in patients suffering from occult malignoma, even after extended diagnostic procedures. The steps to the histological proven diagnosis are presented. The prior diagnoses Scleroderma, Sjögren Syndrome, Polymyalgia rheumatica and rheumatic serositis had to be withdrawn in favour of a small cell lung cancer, an adenocarcinoma of the uterus, a Morbus Hodgkin, a mesothelioma and a malignant fibrous histiocytoma.
