Zusammenfassung

Der Morbus Paget ist oftmals eine undiagnostizierte metabolische Erkrankung des Knochens. Die aus der Erkrankung resultierenden Knochenveränderungen führen zu ausgeprägten Problemen am Skelett. In dem vorliegenden Artikel sollen die chirurgischen Möglichkeiten dargestellt werden. Neben der modernen Frakturversorgung, spielen vor allem die Endoprothetik großer Gelenke, die Umstellungsosteotomien langer Knochen und chirurgische Interventionen an der Wirbelsäule eine Rolle. Neben den allgemeinen Vorbereitungen einer Operation muss immer auch an die Behandlung der zugrundeliegenden Erkrankung mit Bisphosphonaten gedacht werden, um postoperative Komplikationen zu vermeiden.

Summary

Paget’s disease of bone, an often undiagnosed metabolic bone disease, can lead to dramatic skeletal changes with enlargement and bowing of the affected bones. The article gives an overview on recent perspectives on surgical treatment options. Fracture fixation, total joint replacements, osteotomies of long bones and spinal decompression are the most relevant surgical procedures in Pagets disease. Along with preoperative planning of the intervention itself, a pharmacological antipaget treatment with bisphosphonates is mandatory to reduce peri-and postoperative complications.
