Operative Korrektur rheumatischer Destruktionen und Deformierungen der MTP-Gelenke II–V

Zusammenfassung

Anhand pathomechanischer und operations-technischer Überlegungen werden Vorteile des plantaren Zugangs für die Korrektur schwerer rheumatischer Vorfußdeformierun-gen dargestellt. Die Möglichkeiten der Teno -lyse der Beugesehnen sowie der Stellungskorrektur durch Raffung der plantaren Kapsel der MTP-Gelenke und der Haut (Dermodese) wer-den hervorgehoben. Eine eindeutige Überlegenheit des plantaren Zugangs ist der Lite-ratur nicht zu entnehmen. Die Wahl bleibt der Entscheidung des Operateurs überlassen.

Summary

Due to pathomechanical and surgical considerations advantages of a plantar approach for the correction of serious rheumatoid forefoot deformities are presented. The possibilities of tenolysis of the flexor tendons and of reduction of malpositions by drawing tight the plantar capsule of the MTP joints and by dermodesis are emphasized. An obvious superiority of the plantar approach cannot be interfered from the literature. The decision will remain up to the surgeon.
