Zusammenfassung

Das Thema „Psychosen bei Epilepsien” ist paradigmatisch und zukunftsweisend für neuropsychiatrische Fragestellungen und zugleich voller Ungereimtheiten. Zum einen mehren sich die Hinweise darauf, dass psychische Störungen und Epilepsie möglicherweise gemeinsame ätiologische Wurzeln haben. Zum anderen sind Psychosen bei Epilepsie zwar häufig, aber nur selten mit denen aus der Psychiatrie bekannten klinisch übereinstimmend. Sie treten auf bei An-fallshäufung einerseits und bei plötzlicher Anfallsfreiheit andererseits, bei Gabe von Antiepileptika sowie ebenfalls bei deren Entzug. Sie sind also eine Thematik, die zu Thesen und Antithesen Anlass gibt, ohne dass bisher eine plausible und begründbare Synthese entwickelt werden konnte. Psychosen bei Epilepsien sind eine Herausforderung für die Entwicklung einer gemeinsamen neurologisch-psychiatrischen Theorie mit dem Ziel, wissenschaftlichen Fortschritt voranzutreiben und das Wohl der betroffenen Patienten angemessen zu fördern.

Summary

The topic „psychoses in epilepsy” is a paradigm for future neuro-psychiatric research, but it is full of contradictions as well. On the one hand there is growing evidence that psychiatric disorders and epilepsy may have common etiological sources. On the other hand we only find little convergence between psychiatric types of psychoses and epilepsy psychoses, but nevertheless a rather high prevalence of psychoses in patients with epilepsy. Contradictory as well is that the risk of psychosis is raised by high seizure frequencies as well as by a sudden complete seizure stop. They occur with the taking of various anti-epileptic substances as well as with their withdrawal. Thus by their unexplained contradictory features the psychoses of epilepsy can induce the creation of hypotheses and a scientific discussion between the disciplines of neurology and psychiatry for an improvement of scientific progress and for the sake of the affected patients.
