Zusammenfassung
Die vorliegende Übersicht konzentriert sich auf den Krankheitsverlauf und die psychosoziale
Anpassung von Kindern und Jugendlichen mit Zwangsstörungen. Obwohl Zwangsstörungen
im Kindes- und Jugendalter häufig auftreten, ist die Studienlage zum Krankheitsverlauf
allgemein, und noch mehr zur psychosozialen Anpassung der betroffenen Kinder und Jugendlichen,
sehr gering. Die Arbeit gibt eine Übersicht über die Symptomatik und Therapieempfehlungen
und geht vertiefend auf den Verlauf von Zwangsstörungen ein. Einen Schwerpunkt bildet
die Darstellung von psychosozialen Beeinträchtigungen in Schule, Familie und Partnerschaft
sowie in der Freizeit anhand von ausgewählten Studien. Empfohlen wird eine frühe störungsspezifische
Behandlung, die nicht nur für den Verlauf der Zwangsstörung relevant ist, sondern
auch für die psychosoziale Anpassung. Zudem sollte das häufige Auftreten von komorbiden
psychischen Störungen sowie die zusätzliche Stärkung von sozialen Kompetenzen berücksichtigt
werden.
Summary
The review focuses on the course of Obsessive-Compulsive Disorders (OCD) and on aspects
of psychosocial adaption in children and adolescents with OCD. Although OCD and early
onset OCD has a high prevalence, only a few studies on the course and furthermore
less studies on psychosocial adaption in early onset OCD have been published yet.
At first the present review summarizes the symptomatology of OCD and treatment recommendations
for early onset OCD. Then gives an overview including follow up studies and studies
of psychosocial adjustment in early onset OCD considering especially social adaption
in education, family and partnership, as well as in spare time. A conclusion of the
review is that the latency between onset of OCD and consultancy seems to be relevant
not only in the course of symptom development but also for the level of psychosocial
adjustment. A focus in therapy with OCD patients should be laid on OCD specific treatment,
consideration of comorbide psychiatric disorders and on the mediation of social skills.
