Zusammenfassung

Bipolare Störungen gehören zu den häufigen und schwersten psychischen Erkrankungen und stellen eine diagnostische und therapeutische Herausforderung dar. Erste Symptome treten häufig bereits in der Adoleszenz oder im frühen Erwachsenenalter auf. Dennoch wird die Diagnose oft sehr spät gestellt, dadurch wird eine adäquate Behandlung verzögert und die Prognose verschlechtert. In der Behandlung bipolarer Störungen kommt der Pharmakotherapie eine zentrale Bedeutung zu. Dabei muss zwischen der Behandlung akuter manischer oder depressiver Phasen (Akutbehandlung) und der längerfristigen Reduktion bzw. Vermeidung weiterer affektiver Episoden (Phasenprophylaxe) unterschieden werden. In der nachfolgenden Übersicht werden wichtige diagnostische Probleme erörtert und aktuelle pharmakologische Strategien dargestellt.

Summary

Bipolar disorder is one of the most common and most severe mental disorders, constituting a great challenge with respect to both diagnostics and therapy. Even though first symptoms usually occur in adolescence or early adulthood, it is often diagnosed much later, resulting in a great delay of adequate treatment and thereby in a worsened prognosis. Pharmacotherapy plays a primary role in the treatment of bipolar disorder. In this regard, it is important to distinguish between the treatment of acute manic or depressed phases on the one hand (acute treatment) and the longer-term reduction or prevention of further affective episodes on the other (phase prevention). The following overview will discuss important diagnostic problems and illustrate current pharmacological strategies.
