Akute allergische Thrombozytopenien und akute Immunthrombozytopenien durch spezifische Thrombozytenantikörper

Zusammenfassung
Akute allergische Thrombozytopenien können durch Medikamente und durch Nahrungsmittel ausgelöst werden, aber auch parainfektiös und postinfektiös sowie selten nach Bluttransfusionen beobachtet werden. Während bei medikamentös-allergischen Thrombozytopenien die Megakaryo- zyten im Knochenmark eher vermehrt sind, sind sie beim rein toxischen Markschaden dagegen charakteristischerweise vermindert. Es gibt jedoch von dieser Regel abweichende Befunde. Bezüglich der Pathogenese der allergischen Thrombozytopenien sind zu berücksichtigen :
1. Immunkomplexthrombozytopenie
2. Immunthrombozytopenie durch passive Agglutination
3. medikamenteninduzierte Autoimmunthrombozytopenie .
Auch die postinfektiöse Thrombozytopenie wird durch Immunkomplexe hervorgerufen. Die Heparin-indu- zierte Thrombozytopenie entsteht durch immunologische und auch nichtimmunologische Mechanismen.
