Functional and cosmetic results after FAMM flap

Purpose:

The facial artery musculo-mucosal (FAMM) flap is a versatile and reliable method in reconstructing oral and oropharyngeal defects. Since literature does not provide much information concerning the postoperative morbidity of the flap, this study will assess facial function and cosmetic aspects after using the FAMM flap.

Material and Methods:

From January 2013 to January 2019, ten patients received reconstruction using the FAMM flap for defects in the oral cavity or in the oropharynx. The primary excised tumors were all preoperatively assessed as T2 tumors. All patients received simultaneous neck dissection. Patients were reviewed for the maintenance of facial functions, including facial expression, lip and tongue mobility and articulation.

Results:

All patients maintained a sufficient function of the mouth. The tongue mobility remained excellent (4) to good (2). Only in one patient did the articulation suffer from therapy. Sensation of the upper lip decreased in the majority of patients (5), while the perception on the cheek was preserved everywhere. The position of the commissure at rest as well as in the smile remained symmetrical in all patients.

Conclusion:

The FAMM flap is an oncologically well-tolerated and reliable option for the reconstruction of small to medium defects of the oral cavity. In contrast to the submental island flap a neck dissection of the level I can be performed without putting the vascular pedicle at risk. However, patients should be informed about the risk of reduced upper lip sensation.
