18 F-FET PET uptake characteristics of long-term IDH-wildtype glioma survivors

Ziel/Aim IDH-wildtype (IDHwt) gliomas represent the tumor entity with the worst clinical outcome within the 2016 updated WHO classification of brain tumors. Only rare cases present with a long-term survival over several years. Therefore, we aimed to assess the uptake characteristics on dynamic 18F-FET PET, clinical and molecular genetic parameters of long-term IDH-wildtype glioma survivors (LTS) in comparison to short-time survivors (STS).

Methodik/Methods Patients with a de-novo IDH-wildtype glioma (WHO grade III/IV) and 18F-FET PET prior to any further therapy were identified and stratified into LTS (>48 months survival) and STS ( < 15 months survival). Static and dynamic 18F-FET PET parameters (mean/maximal tumor-to-background-ratio (TBRmean/max), biological-tumor-volume (BTV) and minimal-time-to-peak (TTPmin)), clinical parameters (age, sex, Karnofksy-performance-score, diameter and volume of contrast-enhancement on MRI, mode of surgery (resection/biopsy), initial treatment) and molecular genetics (methylation of MGMT-promoter and TERT-promoter) were assessed and compared between LTS and STS.

Ergebnisse/Results Overall, 42 IDHwt glioma patients were included (13 LTS, 29 STS). LTS were significantly younger (median age 50.4 vs. 64.4 years, p < 0.001), showed a higher proportion of MGMT-promoter methylation (84.6 % vs. 48.3 %, p = 0.041) and a smaller median BTV (12.3 vs. 21.5 ml, p = 0.018) on PET compared to STS. TBRmean/max and TTPmin on 18F-FET PET,MRI-derived parameters and all other above mentioned parameters did not significantly differ between LTS and STS (p>0.05 each).

Schlussfolgerungen/Conclusions IDH-wildtype glioma LTS are characterized by a lower age, higher rate of MGMT-promoter methylation as well as a significantly smaller tumor volume on PET compared to STS, while the MRI-based tumor volume does not differ between both groups. Notably, neither uptake intensity nor tracer kinetics on dynamic 18F-FET PET seem not to be associated with LTS.
