Introduction Zenker diverticula are especially in elderly patients a common cause for dysphagia and regugitation, they are usually treated by myotomy. Our case describes the treatment of a small Zenker diverticulum without myotomy.

Case presentation We present the case of a 72 year old patient presenting with dysphagia and regurgitations. Barium swallow with videofluoroscopy showed a small Zenker diverticulum with a maximal length of about 10 mm. During surgery, we found a small diverticulum, however due to the short length of the diverticulum conventional myotomy was not performed. By suction the complete diverticulum could be everted and stapled.

Discussion Stapling of a Zenker diverticulum is a safe and easy alternative to conventional myotomy, especially in a case of a small diverticulum, which can be everted.
