Middle Ear Adenoma

Introduction Adenomas of the middle ear are rare tumors ( MEA) . They are benign tumors and do not metastasize. They may emanate from the epithelial tissue or glandular components of the middle ear mucosa.

These are benign, glandular neoplasms originating in the middle ear mucosa. Hyams and Michaels first described the MEA as an independent clinical entity in 1976, which they called "adenomatous tumors." In the international literature, more than 100 cases have been described so far. The differential diagnosis can be a glomus tumor, a cholesteatoma, a neurinoma, or a Lipoma MEA manifest only at an advanced stage .The incidence of benign primary middle ear tumors are given with approximately 1: 40,000. Recurrence of a MEA has not been described in the literature.

Case We report about a 50-year-old female patient presented with recurrent otalgia and pulse synchronous tinnitus for one year. The otoscopy showed an initially unclear yellowish, smoothly limited, bulging mass in the area of ​​the upper and lower quadrants of the eardrum, without any delineation to the posterior wall of the ear canal. There was no otorrhea Hearing loss, headache, dizziness, paresthesia or taste disorder were not reported by the patient. The images showed a small, 4mm mass. Histopathological examination revealed the diagnosis of adenoma of the middle ear. Via a transmeatal approach, endoscopic removal of the tumor could be performed complete. This was done with the endoscopic instruments like Panetti. For follow-up we recommended the postoperative CT scan of the temporal bone with second-look operation in case of suspicious findings

Conclusion Differential diagnostically, the adenoma can be considered as a tumor in the middle ear. First choice of the therapy is the Surgical removal.
