Reanimation – Die kardiopulmonale Reanimation von Kindern (Paediatric Life Support)

Zusammenfassung

Schwerwiegende Kindernotfälle und insbesondere der kindliche Atem-Kreislauf-Stillstand sind relativ selten, doch besonders zeitkritisch. Im Vergleich zu Erwachsenen stehen bei Kindern hypoxisch bedingte Kreislaufstillstände aufgrund respiratorischer Störungen mit konsekutiver pulsloser elektrischer Aktivität oder Asystolie im Vordergrund. Die aktuellen Leitlinien 2015 des European Resuscitation Council (ERC) zu den lebensrettenden Maßnahmen bei Kindern (Paediatric Life Support, PLS) berücksichtigen einerseits die limitierte wissenschaftliche Evidenz und andererseits die Aspekte der Praktikabilität - bei geringer Routine der meisten Anwender und großen nationalen und lokalen Strukturunterschieden. Besonderes Augenmerk wurde dabei u.a. auf das frühzeitige Erkennen und die effektive Behandlung des kritisch kranken oder verletzten Kindes gelegt, also auf die Vermeidung des Atem-Kreislauf-Stillstands, sowie auf den frühzeitigen Beginn von Ersthelfermaßnahmen. Bei den Algorithmen hat es im Vergleich zu 2010 keine grundlegenden Änderungen gegeben. Die Sequenz ABC (Atemwege, Beatmung, Circulation) wurde beibehalten.

Abstract

In children, severe emergencies and cardiorespiratory arrests in particular are relatively rare but time-critical events. As compared to adults, hypoxic arrests caused by respiratory disorders that may subsequently result in pulseless electrical activity or asystole are more prevalent. The current Paediatric Life Support (PLS) Guidelines 2015 of the European Resuscitation Council (ERC) acknowledge both limited scientific evidence and aspects of practicability. They also take into account the rather limitedpaediatric routine that most providers have as well as national and local infrastructural differences. Particular emphasis was put on early recognition and treatment of a critically ill or injured child, hence the prevention of cardiorespiratory arrest and the early start of lay rescuer interventions. There have been no major changes in the 2010 algorithms, including retention of the ABC sequence (airway, breathing, circulation).
